Your physician or healthcare provider is the best resource for information on your child’s
immunization and health record status. This is a busy time of year for immunizations and
physicals and appointments fill up quickly--please plan ahead.
Remember to bring your original/certified immunization records with you to the clinics.

Care Ring: Provides low cost physical exams. No immunizations. Client base is the underinsured or no insurance. Kindergarten Health Assessment is $50.

CMC North Park: Provides physical exams and immunizations. The clinic sees clients with insurance Medicaid or for a sliding scale fee. There are nurse visits for those students that only require immunizations. Shot nurse available every day.

Charlotte Mecklenburg Schools International/Family Application Center
Offers newly enrolled CMS students coming from another country, the opportunity to receive required immunizations from a School Health RN or Clinical Assistant II. These services are available 5 days/ week.
